*1	Regular means a doctor (including a regident/trainee), and a person working for a commercial supplier in the industry.
*2	Medical Worker means a nurse, a technician and others working for a medical institution, but not a person working for a commercial supplier in the industry. An official certificate or a letter signed by one’s immediate superior must be shown at the on-site registration desk. Also, Student/Graduate Student is required to show a student ID there.
*3	Accompanying Person must be a spouse or a family member of the registered delegate. It is not nessecary to register for the child aged 15 and under.
*4	Date and time: 18:00-, Saturday, March,13, 2010 (Tentative) Venue: "Swan", 1F, the Kyoto International Conference Center Please note that the reception fee can be paid with the Registration fee.

■ Registration Fee Inclusions

Congress registration includes the following:
Regular / Non-doctors

Admission to scientific programs of the congress
Documentation including a book of abstracts
Opening Ceremony and Closing Ceremony

Accompanying Persons

Opening Ceremony and Closing Ceremony
*Accompanying Persons will not be admitted to scientific programs other than the exhibition.

■ Registration Procedure

1)	Early Registration FAX Registration Please download the FAX REGISTRATION FORM. Registration by e-mail, phone or online cannot be accepted.
2)	On-site Registration Make the payment at the registration desk at the venue. You can pay with a credit card or cash.

■ Payment Method

All payments must be made in Japanese yen by bank transfer or with credit card.
No other type of payment will be accepted.

Bank transfer (telegraphic transfer only)
Please note that all transfer charges must be paid by participants.
Remittance must be arranged within 10 days after registration.
Bank account details will appear in the registration form.
[Payment deadline: February 10, 2010 (tentative)]
When arranging a bank transfer,
the following information is required to fill out on the remittance form:

Overseas:	Application number (issued after completion of on-line registration) and participant's name in the message field
Japanese:	Participant's name in the sender's field

Credit card
American Express, Visa, MasterCard, Diners Club and JCB are acceptable.

■ Confirmation

Please print your confirmation out of the registration site.
In case you select credit card payment, you can print it out right after you registered.
If you select bank transfer, printing can be made after the registration desk has confirmed the payment. Registration desk will inform you by e-mail when the printing is possible.

+	ID number and password, to be registered at first login, will be required to access the registration site.
+	Please remember to bring your confirmation to the congress venue to pick up your congress kit upon arrival.

NOTE
+	Confirmation can be printed as many times as you want at anytime during registration by March 14, 2010.

■ Cancellation

Please note that cancellation after the registration is not accepted.
Your registration fee will not be refunded once registered, therefore please be very careful not to make double payment.
