The Role of Heart Team Approach for Patients with CVD; Prevention to Rehabilitation

The treatment of CVD patients during the acute phase has become minimally invasive and the length of hospitalization has been shortened. However, the increasing number of future heart failure patients with underlying ischemic heart disease has caused some concerns. In addition, the current situation is bipolar, involving elderly patients with multiple diseases with disabilities, and people with mild abnormalities who seriously need a long-term self-control. However, the educational programs for CVD patients at very early stage or acute phase of disease are not being performed sufficiently. The benefit of participating in cardiac rehabilitation needs to be more widely publicized in Japan. In this session, we are going to discuss how to improve methods, skills and quality of care for continuous multi-disciplinary approach for various stages of CVD patients.
