Front Line of Sleep Disordered Breathing Management

It is known that a variety of cardiovascular diseases are associated with sleep disordered breathing. Among SDB, obstructive sleep apnea (OSA) contributes to the onset and progression of cardiovascular diseases via complicated process. The basic of OSA treatment is modification of lifestyle. Although the most potent treatment is CPAP, oral appliance can be chosen when CPAP in not tolerable. New treatment measures are being tried. Central sleep apnea, on the other hand, almost always occurs as a consequence of heart failure in the cardiovascular field. CSA associated with heart failure can be improved by the treatment of heart failure itself. In addition, ASV and night oxygen therapy are also applied. However, ASV, the most effective treatment recently turned out to increase cardiovascular mortality in heart failure patients with reduced ejection fraction and CSA dominant SDB. Phrenic nerve-stimulation is showing promise for the treatment of CSA. In this symposium, frontlines of SDB treatment in cardiovascular area will be presented and future strategic direction will be discussed.
