(4) Cure and Heal the Heart with Team: Construction and Practice of New Organization

Team healthcare (or heart team), where multiple jobs demonstrate expertise and cure patients as a team, is an essential concept in cardiovascular care. The most familiar one is cardiac rehabilitation. In the management of patients with arteriosclerotic diseases or heart failure, or after cardiac surgery, cardiac rehabilitation improves patient's QOL, corrects lifestyle such as diet and exercise and enhances adherence to medication. It is the most effective means for promoting social reintegration and preventing reoccurrence. Since less invasive therapeutic options, such as TAVR and MitraClip, came into clinical practice, it is important to discuss what kind of treatment is the best for each patient in the heart team consisting of cardiac surgeon, circulatory physician, anesthesiologist and medical staffs. However, even in hospitals specialized to cardiovascular diseases, there are not many cases where cardiac rehabilitation is not done at all or heart team is not active. It is urgent to establish a new team medical system to cope with the rapidly increasing number patients with cardiovascular disease. In this symposium, we would like to discuss the way of team medicine to correspond various scenes of clinical practice and treatment, the organization of the team to do it, how to operate it, and the result of advanced team medicine. We would like to expect proactive applications from hospitals or institutes which practice advanced initiatives.
